Physical Therapist, Early Intervention

Full Time or Part Time

We are currently seeking a highly motivated certified physical therapist to provide home-based services for children from birth through -3 years old.

Responsibilities:
  • Provide direct patient care and develop individualized treatment plans based on patient needs and goals
  • Conduct evaluations and assessments, if needed, to determine the patient's condition and develop appropriate treatment plans
  • Administer therapeutic exercises, manual therapy techniques, and other interventions to improve mobility, strength, and function
  • Educate patients and their families on proper techniques for exercises and activities to promote recovery and prevent further injury
  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes
  • Collaborate with other healthcare professionals, such as physicians and occupational therapists, to coordinate patient care
  • Maintain accurate and up-to-date documentation of patient evaluations, progress notes, and treatment plans
Qualifications:
  • Bachelor's or master's degree in Physical Therapy from an accredited program
  • Valid state license or certification as a Physical Therapist
  • Knowledge of medical terminology, anatomy, physiology, and pathology related to physical therapy
  • Strong interpersonal skills and the ability to effectively communicate with patients, families, and other healthcare professionals
  • Ability to work independently and as part of a multidisciplinary team
  • Experience working with pediatric patients desired but not mandatory
  • Experience in trigger-point therapy preferred but not required
  • Previous experience working in a hospital or acute care setting is a plus

Occupational Therapist, Early Intervention

Full Time or Part Time

We are currently seeking a highly motivated certified occupational therapist to provide home-based services for children from birth through -3 years old.

Responsibilities:
  • Assess and evaluate patients' physical and mental abilities to determine their occupational therapy needs
  • Develop and implement individualized treatment plans to help patients regain or improve their ability to perform daily activities
  • Provide therapeutic interventions such as exercises, activities, and adaptive equipment to improve patients' functional abilities
  • Monitor and document patients' progress throughout the therapy process
  • Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
  • Educate patients and their families on techniques for managing their conditions and promoting independence
Qualifications:
  • Bachelor’s degree or greater in Occupational Therapy
  • Valid license to practice Occupational Therapy
  • Excellent knowledge of principles and practices of occupational therapy
  • Hands-on experience with computer programs and creative media
  • Familiarity with ADL evaluation tools (e.g., ADL scale, Lawton IADL Scale)
  • Ability to manage patients with different types of personalities
  • Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members
  • Ability to work independently and manage a flexible schedule
  • Access to reliable transportation and a valid driver's license
  • Strong knowledge of physiology and anatomy related to occupational therapy
  • Familiarity with medical imaging techniques and their application in occupational therapy
  • Experience working with pediatric patients and addressing their unique needs
  • Proficiency in speech therapy techniques to address communication challenges
  • Ability to work in critical care settings, including acute care and ICU environments

Speech Pathologist, Early Intervention

Full Time or Part Time

We are currently seeking a highly motivated certified speech pathologist to provide home-based services for children from birth through -3 years old.

Responsibilities:
  • Develop and implement individualized treatment plans for patients
  • Provide therapy sessions to improve speech and language skills
  • Monitor progress and adjust treatment plans as necessary
  • Collaborate with other healthcare professionals, such as doctors and educators, to provide comprehensive care
  • Maintain accurate and up-to-date patient records
  • Provide education and support to patients and their families
Qualifications:
  • Master's degree in Speech-Language Pathology from an accredited program
  • Valid state license or certification in Speech-Language Pathology
  • Previous experience working as a speech-language pathologist preferred
  • Knowledge of diagnostic procedures and treatment techniques for speech, language, and communication disorders
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Excellent problem-solving skills
  • Experience working with pediatric patients preferred
  • Familiarity with critical care and acute care settings, including ICU, if needed
  • Ability to work effectively in a multidisciplinary team

ABA/SI Therapist, Early Intervention

Full Time or Part Time

We are currently seeking a highly motivated certified ABA/SI therapist to provide home-based services for children from birth through -3 years old.

Responsibilities:
  • Delivering 1:1 instruction in accordance with the Individualized Family Service Plan (IFSP)
  • Implementing behavior intervention plans effectively
  • Collecting and graphing daily data to track progress
  • Maintaining detailed documentation of session progress and ABA program book
Qualifications:
  • To be considered for this position, you must have one of the following certifications:
    1. New York State Special Education Certification (Birth–Grade 2)
    2. NYC DOE Special Education Certification (Birth–Grade 2)
    3. LMSW license in the State of New York
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members
  • Ability to work independently and manage a flexible schedule
  • Access to reliable transportation and a valid driver's license
  • Experience working with pediatric patients and addressing their unique needs

Early Intervention Service Coordinator

Full Time or Part Time

We are currently seeking a highly motivated service coordinator to coordinate home-based services for children from birth through -3 years old.

Responsibilities:
  • Assign therapists to referred cases
  • Schedule therapy appointments
  • Engage with therapists, patients, and families to ensure effective staffing
  • Respond to calls and email from families receiving services
Qualifications:
  • All early intervention service coordinators, whether individual service coordinators or employees or subcontractors of an approved provider of service coordination services, are required to meet a minimum of one of the following educational or service coordination experience credentials:
    1. Two years of experience in service coordination activities as delineated in regulation (voluntary or part-time experience that can be verified and will be accepted on a pro rata basis)
    2. One year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities
    3. One year of service coordination experience and an associate degree in a health or human service field, or a bachelor's degree in a health or human service field
  • Excellent communication skills to effectively interact with patients, families, and healthcare team members
  • Ability to work independently and manage a flexible schedule
  • Access to reliable transportation and a valid driver's license
  • Experience working with pediatric patients and addressing their unique needs
